摘　　要： 目的探讨降钙素原（PCT）在剖宫产术后合并切口感染致病菌类别的早期诊断价值。方法回顾性分析2012年1月-2015年12月收治的行剖宫产术的产妇，其中合并术后切口感染的产妇38例，革兰氏染色阳性（G^＋组）30例和革兰氏染色阴性（G^＋组）8例，随机选取未感染的产妇38例设为对照组，比较其术后第1、3、7天PCT和CRP的水平。结果感染纽和对照组术后第1天PCT和CRP均有升高，但差异无统计学意义（P〉0．05）；术后3dPCT和C-反应蛋白（CRP）明显升高，差异有统计学意义（P〈0．05）；不同病原菌感染引起的PCT浓度不同，阳性菌浓度大于阴性菌浓度，差异有统计学意义（P〈0．05）；,而CRP浓度差异无统计学意义（P〉0．05）。结论剖宫产术后感染病原菌的种类与PCT的浓度有一定的相关性，在细菌培养和药敏报告前检测PCT的浓度能指导临床合理选用抗生素。 Objective To evaluate the procalcitonin （ PCT ） in patients with cesarean section incision infection of early diagnosis. Methods A retrospective study was done on patients underwent cesarean section from January 2012 to December 2015 in Obstetrics Gynecology. The patients were divided into infection group （ 38cases ） that contained gram positive （ G^＋ ） 30 cases and gram negative （ G^＋ ） 8 cases and control group （ 38cases ） . And then the 1st, 3nd and 7th day levels of PCT and CRP were compared. Results Comparison of observation group to control group showed no significant differences in time of 1 day after operation of lever of PCT and CRP （ P〉0.05 ） .But the levels of PCT and CRP were rise obviously after 3 day （ P〈0.05 ） .The PCT levels of patients after operation infection were various .The gram positive group was lower than the gram negative groups （ P〈0.05 ） .However CRP values were similar （ P〉0.05 ） . Conclusion The kinds of pathogenic bacteria infection after cesarean section and the concentration of PCT has certain relevance.It can detect the concentration of PCT to guide clinical rational use of antibiotics in front of the bacteria culture and drug susceptibility returns.
