摘　　要： 目的探讨血清高尔基蛋白73（GP73）及脱-γ-羧基凝血酶原（DCP）水平用于慢性乙型肝炎病毒（HBV）相关肝细胞癌（HCC）诊断的临床价值。方法 纳入2014年8月至2016年4月就诊的HBV相关HCC患者126例，同时设置肝硬化患者（n=96）及慢性乙型肝炎（CHB）患者（n=68）作为对照。患者就诊时记录一般情况及常规实验室指标，检测血清GP73、DCP及AFP水平。ROC曲线检测GP73、DCPTJtAFP单独或联合用于HCC诊断的价值。结果血清GP73水平由高至低依次为HCC组、肝硬化组及cHB组；DCP水平由高至低依次为HCC组、肝硬化组7LCHB组。HCC组中，临床分期分别与血清AFP、GP737LDCP水平呈正相关，AFP与GP737及DCP呈正相关。GP73单独诊断的AUC为0．790，DCP的AUC为0．752，AFP联合GP737ADCP的AUC为0．869。结论血清GP737LDCP水平在HBV相关HCC中显著升高且与临床分期正相关，GP73、DCP及-AFP联合诊断HCC的临床价值较高。 Objective To investigate the clinical diagnostic value of serum Golgi protein-73 （ GP73 ） and Des- γ -carboxy prothrombin （ DCP ） levels in patients with HBV-related HCC. Methods A total of 126 patients with HBV-related HCC from August 2014 to April 2016 in our hospital were enrolled, and 96 cirrhosis （ n=96 ） and chronic hepatitis B （ CHB ） （ n=68 ） patients were enrolled as control. The general information and laboratory parameters were recorded, and the serum GP73, DCP and AFP levels were tested. The correlations between Persepsin and parameters were analyzed, and diagnostic efficiency of GP73 and DCP for HCC in patients infected with HBV was calculated by ROC curve. Results Serum GP73 levels were divided into HCC group, cirrhosis group and CHB group from high to low. The DCP levels were divided into HCC group, cirrhosis group and CHB group from high to low. In HCC group, the clinical stage was significantly positively correlated with AFP, GP73 and DCP.The AFP level was positively correlated with GP73 and DCP. AUC of GP73 used for the diagnosis of HCC was 0.790, the AUC of DCP was 0.752, and the AUC of GP73 combined with DCP was 0.869. Conclusion Serum GP73 and DCP levels are elevated in patient with HBV-related HCC, and they can be used to improve the diagnosis of HCC combined with AFP.
