连续性血液滤过、血液灌流联合治疗对长期血液透析患者的疗效和炎性因子的影响

摘  要: 目的探讨连续性血液滤过、血液灌流联合治疗对长期血液透析患者的临床疗效和炎性因子的影响。方法收集2015年5月至2017年5月入院的80例长期血液透析患者,按随机数字表法分为试验组和对照组各40例,对照组给予连续性血液滤过治疗,实验组则加施血液灌流,比较两组患者疾病相关参数、总体治疗效果与炎症因子。结果实验组治疗后体温、心率、呼吸频率、白细胞、血红蛋白、谷丙转氨酶(ALT)、谷草转氨酶(AST)、血总胆红素(TB)、血肌酐、尿素氮、血钠、降钙素原(PCT)、C反应蛋白(CRP)、肿癌坏死因子-α(TNF-α)、白细胞介素-6(IL-6)与β2微球蛋白(β2-MG)水平均低于对照组,平均动脉压、氧合指数及总有效率均高于对照组(P<0.05)。结论连续性血液滤过、血液灌流联合治疗对长期血液透析患者的临床疗效和炎性因子的影响显著,具有借鉴意义。 Objective To investigate the effects of combined continuous hemofiltration with hemoperfusion on the clinical effi- cacy and inflammatory factors in patients with long-term hemodialysis. Methods Eighty patients with long-term hemodialysis admitted from May 2015 to May 2017 were randomly divided into experimental and control groups. The control group received continuous hemofil- tration. The experimental group was treated with combined therapy of hemofihration and hemoperfusion. Disease-related parameters, o- verall therapeutic effect and inflammatory factors were compared between the two groups. Results After the treatments,the body tem- perature, heart rate, respiratory rate, white blood cells, hemoglobin, ALT, AST, blood TB, serum creatinine, urea nitrogen, serum sodium, PCT, CRP ,TNF-αβ IL-6 and β2-MG in the experimental groups was lower than those in the control group (P 〈0. 01 ). The total effec- tive rate, the mean arterial pressure and the oxygenation index in the experimental group was significantly higher than that of the control group ( P 〈0. 05 ). Conclusion The combined therapy of Continuous hemofihration and hemoperfusion has significant effect on the clinical efficacy and inflammatory factors of patients with long-term hemodialysis.
