摘　　要： 目的:探讨寰枢椎脱位的复杂颅颈交界区畸形后路减压复位内固定术疗效初步观察及对MRI扫描椎管矢径的影响。方法:选取2014年1月-2016年12月于南昌大学第二附属医院及南昌大学附属九江医院诊治的寰枢椎脱位患者40例作为研究对象,采用随机数表法分为对照组和研究组,每组20例,对照组应用经枕颈融合术治疗,研究组应用复位内固定术治疗。比较两组患者手术指标及手术前后不同时间点JOA评分。结果:研究组手术时间、住院时间、术中出血量以及下床活动时间均短于对照组,差异均有统计学意义(P<0.05);治疗前两组JOA评分比较,差异无统计学意义(P>0.05),治疗后两组均得到一定改善,研究组各个时间点JOA评分均优于对照组,差异均有统计学意义(P<0.05);治疗前两组MRI扫描椎管矢径相关指标比较,差异均无统计学意义(P>0.05),治疗后均得到一定改善,研究组各项椎管矢径相关指标均优于对照组,差异均有统计学意义(P<0.05)。结论:寰枢椎脱位畸形结构比较复杂,需要对患者做好评估工作,熟悉解剖结构并在此基础上选用个性化的治疗技术,从而改善治疗效果。 Objective ： To investigate the curative effect of atlantoaxial dislocation complex cranial neck deformity posterior decompression reduction internal fixation and its effect for MRI scan vertebral canal sagittal diameter.Method： From Janum7 2014 to December 2016, 40 patients with atlantoaxial dislocation treated in the Second Affiliated Hospital of Nanchang University and Jiujiang Affiliated Hospital of Nanchang University were selected as the retrospective study objects and divided into the control group and the study group according the random number table method, the control group was given the pillow cervical arthrodesis, the study group was given reduction internal fixation.Operation indicators and JOA scores at the different time of two groups were compared.Result： The operation time, hospital stay and operation bleeding of the study group were lower than those of the control group, the differences were statistically significant （ P〈0.05 ） .There was no significant difference in J（）A scores of two groups before treatment （ P〉0.05 ）, after treatment, JOA scores of two groups were improved, and JOA scores at different times of the study group were better than those of the control group, the differences were statistically significant （ P〈0.05 ） .There were no significant differences between vertebral canal sagittal diameter related indicators of two groups before treatment （ P〉0.05 ）, after treatment, vertebral canal sagittal diameter related indicators of two groups were improved, and the study group were better than those of the control group, the differences were statistically significant （P〈0.05） .Conclusion： Atlantoaxial dislocation deformed vertebral structure is complex, need thorough assessment for patient, choose personalized treatment technology on the basis of doctor familiar with the anatomy, in order to improve treatment effect.
