摘　　要： 目的检测杭州地区育龄妇女巨细胞病毒(CMV)IgM、IgG抗体,选取结果为双阳性者进行IgG抗体亲合力分析,探讨其对于CMV原发性感染诊断的临床意义。方法用化学发光微粒子免疫检测法(CMIA)检测血清中的抗巨细胞病毒IgM/IgG抗体(CMV IgM/IgG),并方便选取2014年1月—2016年12月该院临床送检TORCH抗体检测血清标本中CMV IgM和IgG均为阳性的育龄妇女(15~49岁)共163例,进行IgG亲合力检测,计算亲合力指数(avidity index,AI)。结果 2014—2016年该院育龄妇女CMV IgM、IgG都为阳性的标本共163例,同时检测其CMV IgG亲合力,亲合力指数为10.47%~99.90%,平均为89.49%,其中AI低于50%有4例,占比2.45%,为低亲合力IgG抗体;AI高于60%有158例,占96.93%,为高亲合力抗体;AI介于50%~60%之间有1例,占0.61%,为灰区值。结论杭州地区育龄妇女中CMV IgM阳性的患者大部分不是近期原发感染,结合CMV IgM/IgG检测及IgG亲合力分析可以大大提高检测效率。 Objective To test the IgG avidity index of child-bearing women with positive IgM and IgG antibodies of cytomegaloviruses in Hangzhou and study the clinical significance of it to the CMV primary infection diagnosis. Methods The CMV IgM/IgG in serum was tested by the CMIA and the 163 cases of children-bearing women aged from 15 to 49 whose CMV IgM and IgG in serum were positive from January 2014 to December 2016 were convenient selected for IgG avidity test and the AI was calculated. Results There were 163 cases of child-bearing women in our hospital from 2014 to 2016 whose CMV IgM and IgG were positive and the CMV IgG avidity was tested and the avidity index was 10.47% ~99.90%,89.49% on average, and the AI less than 50% was in 4 cases, accounting for 2.45%, belonging to the low avidity antibody,and there were 158 cases whose AI was more than 60%, accounting for 96.93%, belonging to the high avidity antibody, and there was 1 case whose AI was between 50% and 60%, accounting for 0.61%, and it is grey zone value. Conclusion Most child-bearing women in Hangzhou whose CMV IgM was positive is not the short-term primary infection, and the CMV IgM/IgG test and IgG avidity analysis can greatly improve the test efficacy.
