摘　　要： 目的探讨小梁切除联合超声乳化术治疗原发性闭角型青光眼合并白内障的临床疗效。方法研究对象选自在2013年1月—2017年1月阶段该科室收治的原发性闭角型青光眼合并白内障患者92例共106患眼,将入选的研究对象进行随机分组,分为两组进行对照研究,即观察组(46例患者共52患眼)和对照组(46例患者54患眼)。对照组患者行小梁切除术,观察组患者行小梁切除联合超声乳化术进行治疗。结果两组患者的术前眼压水平比较,经统计学分析差异无统计学意义(P>0.05)。观察组患者术后眼压,明显低于对照组,经统计学分析差异有统计学意义(P<0.05)。两组患者的术前视力、中央前房深度、周边前房深度的比较,经统计学分析差异无统计学意义(P>0.05)。观察组患者术后视力明显优于对照组,中央前房深度明显大于对照组,周边前房宽度明显大于对照组,经统计学分析差异有统计学意义(P<0.05)。观察组患者的角膜水肿、前房纤维素渗出等并发症发生率,与对照组患者比较,经统计学分析差异无统计学意义(P>0.05)。观察组患者的手术成功率为98.08%,明显高于对照组的87.04%,经统计学分析差异有统计学意义(χ~2=13.673,P=0.018)。结论小梁切除联合超声乳化术治疗原发性闭角型青光眼合并白内障,能够显著降低患者术后眼压,提高患者术后视力,显著提高患者的手术治疗成功率,且不会增加患者的手术并发症,具有较高的临床应用价值。 Objective To study the clinical effect of trabeculectomy and phacoemulsification in treatment of primary angle closure glaucoma and cataract. Methods 92 cases(106 affected eyes) with primary angle closure glaucoma and cataract admitted and treated in our department from January 2013 to January 2017 were convenient selected and randomly divided into two groups with 46 cases( 52 affected eyes) in each, the control group adopted the trabeculectomy, while the observation group were treated with trabeculectomy and phacoemulsification. Results The difference in the preoperative eye pressure level between the two groups was not obvious(P>0.05), and the postoperative eye pressure in the observation group was obviously lower than that in the control group and the difference was obvious(P<0.05), and the differences in the preoperative vision, central anterior chamber depth and surrounding anterior chamber depth between the two groups were not obvious, and the surrounding anterior chamber width was bigger than that in the control group, and the difference was obvious(P<0.05), and the difference in the incidence rate of complications between the two groups was not obvious(P>0.05) and the success rate of operation in the observation group was obviously higher than that in the control group, and the difference was obvious with statistical significance,(98.08% vs 87.04%),(χ~2=13.673,P=0.018). Conclusion The trabeculectomy and phacoemulsification in treatment of primary angle closure glaucoma and cataract can obviously reduce the postoperative eye pressure of patients, improve the postoperative vision, obviously improve the treatment success rate without increasing the operative complications and it is of higher clinical application value.
