摘　　要： 目的:运用示剂追踪法行腹部CT增强扫描,观察动脉期触发扫描时间,为CT增强扫描准确把握动脉期影像给出合理建议。方法:收集224例临床疑诊腹部病变而行腹部CT增强扫描的检查者,随机分为二组,A组112例使用浓度为300 mg I·m L^(-1)碘海醇注射液;B组112例使用浓度为350 mg I·m L^(-1)碘海醇注射液。所有检查者均行常规腹部动态三期扫描,从肘正中静脉置入22 G留置针,以3 m L·s^(-1)速率注射对比剂,增强扫描采用示剂追踪法完成,设置腹主动脉膈肌层面为兴趣区,触发扫描HU值为150 HU,自动触发扫描。结果:A组触发扫描时间为13~46 s,平均(22.75±4.79)s;B组触发扫描时间为12~41 s,平均(21.8±5.1)s,两组比较差异无统计学意义(P=0.242);同时测量腹主动脉CT值,两种不同浓度对比剂在注射速率相同情况下,两组CT值的比较,差异无统计学意义(P=0.436)。结论:在64排螺旋CT动态增强扫描时,对比剂注射速率3 m L·s^(-1),对比剂浓度分别为300 mg I·m L^(-1)与350 mg I·m L^(-1)时进行腹部增强扫描,示剂追踪法动脉期扫描触发均值时间在21~22 s,实际扫描均值时间27~28 s,与相关教科书提出的25~30 s扫描动脉期时间一致。但示剂追踪法实际扫描最短18 s,最长52 s,更符合个性化扫描要求,因此建议在带有示剂追踪扫描软件的CT扫描设备进行腹部动态增强扫描时,采用示剂追踪法能更准确的抓住动脉期。 Objective: To observe the duration of arterial phase triggering by abutment CT scan,and to make reasonable suggestions for accurately grasping arterial phase images by CT enhanced scan. Methods: A total of 224 patients with clinically suspected abdominal lesions who underwent abdominal CT scan were randomly divided into two groups: group A( n = 112) was treated with 300 mg I · m L^(-1)iohexol injection. Group B( n = 112) with concentration of 350 mg I ·m L^(-1)iohexol injection. All the patients underwent routine abdominal dynamic three-phase scan. The 22 G detaining needle were placed from the middle of the elbow of the patients,and the contrast agent was injected at a rate of 3 m L · s^(-1);to rate 3 m L · s^(-1)injection of contrast agent enhancement scanning with show agent tracking method and abdominal aorta diaphragm level set as the region of interest( ROI),manually triggered scan for 150 HU. The enhanced scan was performed by the method of tracing. The abdominal aortic diaphragm was set as the region of interest( ROI) to trigger the scan HU value to 150 HU,then,automatically trigger the scan. Results: The scanning time of group A was 13 ~ 46 s and the average was( 22. 75 ± 4. 79) s. The scanning time of group B was 12 ~ 41 s and the average was( 21. 8 ± 5. 1) s.There was no statistically significant difference. There was no significant differenc between the two groups( P = 0. 242). At the same time,the CT value of abdominal aorta was measured. There was no significant difference in the CT values between the two groups at the same injection rate of contrast agents with different concentration( P = 0. 436). Conclusion:In the 64-row spiral CT dynamic enhanced scan with the contrast agent injection rate of 3 m L·s^(-1),abdominal enhanced san was carried out with 300 mg I·m L^(-1)and 350 mg I·m L^(-1)of concentration of contrast agent respectively,The mean time of the arterial phase scanning triggered by tracer tracing was 21 ~ 22 s,and the actual scan mean time was 27 ~ 28 s,which was consistent with
