MRI扫描联合血清GP73、AFP水平检测对乙型肝炎肝硬化背景小肝癌诊断的研究

摘  要: 目的研究磁共振成像(magnetic resonance imaging,MRI)扫描联合血清高尔基体膜蛋白73(Golgi protein 73,GP73)、甲胎蛋白(alpha fetoprotein,AFP)水平检测在诊断乙型肝炎肝硬化背景小肝癌(small hepatocellular carcinoma,SHCC)中的临床意义。方法将2016年3月至2016年10月于本院住院的62例乙型肝炎肝硬化背景SHCC患者纳入观察组,选取同期本院60例良性肝病患者纳入肝病对照组,60例门诊健康体检者纳入健康对照组。检测三组研究对象血清GP73和AFP水平,回顾性分析观察组患者MRI扫描结果,并比较三种指标对SHCC的诊断价值。结果观察组患者血清GP73和AFP水平均显著高于肝病对照组和健康对照组(P<0.05)。肝病对照组患者血清GP73和AFP水平均显著高于健康对照组(P<0.05)。MRI扫描对SHCC诊断的灵敏度、特异度及准确度均显著高于血清GP73和AFP水平检测(P<0.017)。血清GP73和AFP水平检测联合MRI扫描对SHCC诊断的灵敏度、特异度及准确度均显著高于其他4种检测方法(P<0.005)。结论 MRI扫描联合血清GP73和AFP水平检测对SHCC的诊断具有重要价值,可以明显提高SHCC的检出率和诊断的准确性。 Objective To study the effectiveness of using magnetic resonance imaging (MRI) combined with serum Golgi protein 73 (GP73) and alpha fetoprotein (AFP) assay to diagnose small hepatocellular carcinoma (SHCC) in hepatitis B patients with cirrhosis.Method 62 SHCC in hepatitis B patients with cirrhosi treated in our hospital from March 2016 to October 2016 were enrolled in observation group, 60 patients with benign liver diseases were enrolled in liver disease control group, and 60 healthy people with physical examination in outpatient department were enrolled in healthy control group. The serum levels of GP73 and AFP were detected among the three groups. The results of MRI scanning in observation group were retrospectively analyzed, and the diagnostic value of the three indexes for SHCC were compared.Result The serum levels of GP73 and AFP in observation group were significantly higher than those in the liver disease control group and healthy control group (P〈0.05). The serum levels of GP73 and AFP in liver disease control group were significantly higher than those in healthy control group (P〈0.05). The sensitivity, specificity and accuracy of MRI scan for SHCC diagnosis were significantly higher than those of serum levels of GP73 and AFP (P〈0.017). The sensitivity, specificity and accuracy of combined use of serum levels of GP73 and AFP combined with MRI scan for SHCC diagnosis were significantly higher than those of the other 4 methods (P〈0.005).Conclusion MRI scanning combined with serum levels of AFP and GP73 is very effective in detecting SHCC, and can increases detection rate and diagnosis accuracy.
