摘　　要： 目的观察中西医结合治疗缺血性进展性卒中的临床疗效及对血清C反应蛋白、血浆纤维蛋白原的影响。方法将220例进展性卒中病人随机分为治疗组和对照组,每组110例。对照组给予西医治疗,同时调控血压、血脂、血糖以及康复治疗。治疗组在西医治疗的基础上进行中医辨证,分为风痰阻络型、气虚血瘀型、痰热腑实型、阴虚风动型并给予相应的中药汤剂治疗。两组均以21d为1个疗程。结果治疗组总有效率为92.7%,对照组总有效率为81.8%,治疗组疗效优于对照组(P<0.05)。两组治疗后美国国立卫生研究院卒中量表(NIHSS)评分较治疗前均降低,治疗组降低程度优于对照组(P<0.05)。治疗后两组血清C反应蛋白和血浆纤维蛋白原均较治疗前明显降低(P<0.05),且治疗组降低水平显著低于对照组(P<0.05)。结论缺血性进展性卒中在西医规范化治疗的基础上进行中医辨证治疗可明显提高临床疗效,改善神经功能缺损程度,降低血清C反应蛋白和血浆纤维蛋白原水平。 Objective To observe the clinical effect of integrated traditional Chinese and Western medicine in the treatment of ischemic stroke and its effect on the serum C-reactive protein and fibrinogen.Methods Two hundreds and twenty patients with progressive stroke were randomly divided into treatment group（n =110）and control group（n =110）.The patients in control group was given western medicine treatment,and regulating blood pressure,blood lipid,blood glucose and rehabilitation therapy.The patients in treatment group on the basis of Western medicine,divided into the wind phlegm type,Qi deficiency and blood stasis,phlegm heat,yin deficiency and give the decoction of Chinese medicine treatment for 21 days.Results Markedly effective rate was 92.7% in treatment group with combination of TCM and Western medicine.The control group markedly effective rate was 81.8%.The treatment group was better than the control group.After treatment,the NIHSS scores of the two groups were lower than those before treatment,and the treatment group was better than the control group（P〈0.05）.The serum levels of C reactive protein and fibrinogen in the two groups were significantly lower than that before treatment（P〈0.05）,and CRP the treatment group was significantly lower than that of the control group（P〈0.05）.Conclution Treatment of ischemic stroke by standardized treatment of Western medicine treatment with TCM can significantly improve the clinical efficacy,improve the degree of neurological impairment,reduce serum C reactive protein and fibrinogen.
