64层螺旋CT检查评价冠状动脉斑块稳定性的价值研究

摘  要: 目的探讨64层螺旋CT在冠状动脉斑块稳定性中的评价价值。方法选取冠心痫患者50例为研究对象,均接受冠状动脉CT血管成像,观察64排螺旋CT冠状动脉成像对各型冠状动脉斑块显示情况,分析不同分级心绞痛患者各型斑块分布情况。测址并比较冠状动脉CT值及延迟期冠状动脉斑块CT值降低值。结果64排螺旋CT冠状动脉成像共显示斑块85处,其中左前降支29处,左旋支26处,左主干15处,右冠15处。Ⅰ型斑块30处,Ⅱ型斑块25处,Ⅲ型斑块30处。Ⅰ级、Ⅱ级、Ⅲ级心绞痛患者各型斑块比例间比较,差异有统计学意义(χ2=14.465,P〈0.05)。组间比较,Ⅰ级心绞痛患者与Ⅱ级、Ⅲ级心绞痛患者各型斑块比例间比较,差异均有统计学意义(χ2=8.878、8.892,均P〈0.05)。Ⅰ级、Ⅱ级、Ⅲ级心绞痛患者冠状动脉斑块CT值[(52.1±12.6)Hu比(29.1±7.7)Hu比(41.6±10.9)Hu]及延迟期CT值降低值[(13.4±3.9)Hu比(7.7±1.6)Hu比(10.4±2.9)Hu]间比较,差异均有统计学意义(F=23.462、18.289,均P〈0.05)。其中Ⅱ级心绞痛患者冠状动脉斑块CT值及延迟期CT值降低值最低。结论在冠状动脉斑块稳定性的评价中,64排螺旋CT具有较离的应用价值。 Objective To evaluate the clinical value of 64 slice spiral CT in coronary plaque stability. Methods 50 patients with coronary heart disease (CHD) who received coronary 'artery CT angiography were selected. The situation of coronary artery plaque by 64 slice spiral CT coronary angiography were observed. The distribution of different types of plaque in patients with different grades of angina pectoris was analyzed. The CT value of coronary. artery and the value of CT value of coronary artery plaque Ⅲ delayed phase were measured and compared. Results 64 slice spiral CT coronary angiography showed a total of 85 plaques. The left anterior descending branch 29, left circumflex branch of the 26 ,the left main trunk of the 15 ,the right crown of the 15. There were 30 types Ⅰ plaques, 25 type Ⅱ plaques,and typeⅢplaques of 30 sites. Comparison of different types of plaques in patients with different grade, the difference was statistically significant ( χ2 = 14. 465, P 〈 0.05 ). Comparison between the groups, the proportion of different types of plaque in patients with angina peetoris and different grade ,the diflerenee was statistically significant ( χ2 = 8. 878,8. 892, all P 〈 0. 05 ). The differences of CT value [ (52.1 ± 12.6 ) Hu vs. ( 29.1 ±7.7 ) Hu vs. (41.6 ± 10.9) Hul and CT value of coronary artery plaque[ ( 13.4 ± 3.9) Hu vs. (7.7 ± 1.6) Hu vs. ( 10.4 ± 2.9)Hu]in patients with grade Ⅰ , Ⅱ and m were statistically significant (F=23.462,18. 289,all P 〈0.05). The CT value and CT value of coronary artery plaque in the patients with angina pectoris were the lowest. Conclusion In the evaluation of coronary plaque stability,64 row spiral CT has high application value.
