瘀血对过敏性紫癜患儿肾脏损伤的影响

摘  要: 目的:通过对过敏性紫癜(HSP)与紫癜性肾炎(HSPN)患儿瘀血积分、凝血相关指标的观察,探讨瘀血对HSP患儿肾脏损伤的影响。方法:85例患儿按有无尿检异常分为HSP组45例,HSPN组40例。根据中医瘀血症候积分表进行瘀血评分。采用免疫比浊法测D-二聚体(D-Dimer),ELISA法测血浆与尿液纤溶酶原激活物抑制剂-1(PAI-1)。结果:与HSP组比较,HSPN组瘀血积分、尿PAI-1/Cr明显增高(P<0.05)。血D-Dimer、血PAI-1在HSP和HSPN组差异无统计学意义。尿PAI-1/Cr与瘀血积分、24h尿蛋白定量均呈正相关(P<0.05)。结论:瘀血参与了HSP患儿肾脏损伤的发生,尿液中PAI-1水平的增高与HSP肾脏损伤有关。 Objective: To explore the effects of blood stasis on renal injury of Henoch-Sch?nlein purpura(HSP) children through the observation of blood stasis score and related indicators of blood coagulation in children with HSP and HenochSch?nlein purpura nephritis(HSPN). Methods: Eighty-five cases of HSP children were divided into HSP group(45 case) and HSPN(40 case) group according to the urine test. The blood stasis was scored according to the TCM blood stasis symptom scale. D-Dimer, plasma and urine plasminogen activator inhibitor-1(PAI-1) were detected by immunoturbidimetry and ELISA method respectively. Results: Compared with HSP group, blood stasis score and urine PAI-1/Cr were obviously elevated in HSPN group(P<0.05). The levels of blood D-Dimer and PAI-1 were not statistically different between HSP and HSPN groups.Urine PAI-1/Cr was positively related to blood stasis score and 24h-urine protein quantity in children with HSPN(P<0.01). Conclusion: Blood stasis was involved in renal injury of children with HSP, and the level of urine PAI-1 was associated with renal injury in children with HSP.
