摘　　要： 目的研究维持性血液透析(maintenance hemodialysis,MHD)患者血浆脂蛋白相关磷脂酶A2(lipoprotein-associated phospholipase A2,Lp-PLA2)与左心室肥厚(left ventricular hypertrophy,LVH)的关系。方法选取MHD患者54例,收集一般情况、生化结果及心脏彩色多普勒超声结果,按照左心室心肌质量指数(left ventricular mass index,LVMI)分为左心室肥厚组(n=33)和左心室正常组(n=21),对比两组患者Lp-PLA2的变化,使用Spearman秩相关和多元线性回归分析评价影响LVH的危险因素。结果左心室肥厚组Lp-PLA2水平显著高于左心室正常组(P<0.01)。Spearman秩相关提示LVMI与Lp-PLA2呈正相关(P<0.01)。多元线性回归分析提示Lp-PLA2是影响LVMI的独立危险因素(P<0.01)。结论 MHD患者LVH与血浆Lp-PLA2相关,血浆Lp-PLA2水平升高可能参与了MHD患者LVH的发生发展。 Objective To investigate whether left ventricular hypertrophy(LVH)accelerated by Lipoprotein-associated Phospholipase A2(Lp-PLA2)in maintenance hemodialysis(MHD)patients.Methods A total of 54 MHD patients were enrolled in this study.General information,laboratory measurements and ultrasonography data for LVH were collected.Patients were divided into two groups,the patients with LVH(n=33),and the patients without LVH(n=21)according to left ventricular mass index(LVMI).Lp-PLA2 was analyzed between the two groups,and the risk factors affecting LVH were analyzed using Spearman rank correlation and multivariate linear regression.Results Higher plasma Lp-PLA2 levels were found in patients with LVH as compared with those in patients without LVH(P<0.01).Spearman rank correlation indicated that LVH was positively correlated with Lp-PLA2(P<0.01).Multivariate linear regression also showed that Lp-PLA2 correlated with LVH(P<0.01).Conclusion LVH is associated with increase of plasma Lp-PLA2 in MHD patients.The elevated levels of Lp-PLA2 may be involved in the occurrence and progression of LVH for MHD patients.
