摘　　要： 县级医院是我国公立医院的重要组成部分,其改革成功与否关系到国家整个医疗体系改革的成败。目前国家正以史无前例的决心和力量加强县级医院的建设。本文结合医疗服务的特性和国内实情,分析了加强县级医院建设中需要通过调整、整合和补偿、投入并举的方法一并解决历史遗留问题的意义,并从顶层设计的高度,建议按系统工程建设的思想,将县级医院提升到由地市(包括副省级市)政府具体统一规划、统筹主办。最终与省、县两级政府协同分别主办三级、二级和一级医疗服务,构建出一个与国际接轨的三层级国家医疗服务体系。 County hospitals are the important portion in Chinese public hospital system.Their reform is very crucial in the reform of national healthcare system. The central government has reinforced the development of national county hospitals than ever before. Based on the characteristics of medical service and Chinese conditions, this paper studied the significance of using integrative strategy to promote the national county hospitals, such as conjunct the policy of regulation or integrate and finance assistance or investment to enhance the national county hospitals＇reform. And further, according to the thinking of systems engineering, this paper suggested that the county hospitals should be directly planned and managed by the municipal government. And then, the county government is just responsible for primary care and public health in its district,the provincial government running the tertiary care （hospital）. This designed structure of national medical care is not only scientific but also meet the international conventions.
