摘　　要： 从覆盖率、公平性、可及性、人口健康、医疗卫生费用五个指标考察不同历史时期的农村合作医疗制度绩效,可以发现:正在实施的新型农村合作医疗制度的绩效虽尚未完全显现,但部分主要指标较高;计划经济时期的农村合作医疗制度绩效次之;而转轨时期的农村合作医疗制度绩效最低。导致不同时期农村合作医疗制度绩效差异的主要原因是政府与农民的不同行为方式。今后要进一步提高新型农村合作医疗制度的绩效,应该着力引导医疗卫生资源向农村配置;完善农村三级医疗卫生服务体系;规范政府财政投入机制;做好城乡一体化的医疗保障制度的有效衔接。 This paper aims to measure efficiency of the Rural Cooperative Medical Care System （RCMS） in different historical periods by five indexes, i.e. coverage rate, impartiality, availability, health and medical sanitation expenses. The efficiency of the current New RCMS has not unfolded completely, but some main indexes are relatively high; the efficiency of the RCMS in the period of planned economy is lower; that of the transitional period is the lowest. The main reason why efficiency rate varies in different periods is due to different behavioral modes of the government and farmers. In order to promote the efficiency of the New RCMS, medical sanitation resources should be channeled into rural areas; the rural three-level medical service network should be improved; the government financial appropriation system should be standardized; and the medical care system should be effectively linked in the process of integrating urban and rural areas.
