摘　　要： ①目的　结合鼻内镜筛窦手术的实际应用需要,对筛窦安全标志进行解剖学测量,为临床手术防止和减少各种严重并发症的发生提供可靠的解剖学资料。②方法　选择30例(60侧)外观无异常、经 40 g/L甲醛固定的成人尸头(男23例,女7例),用MJ 3435型带锯机,沿眉弓上缘及枕骨粗隆上1 cm连线水平锯开颅盖骨,去除脑组织;沿正中线矢状锯开头颅,去除鼻中隔;经眶下孔与正中剖面平行矢状锯开,用解剖刀、筛泡钳和剥离器等依鼻内镜下筛窦手术的相同体位和路径,逐层解剖并测量各解剖结构的径线。③结果　鼻丘中心至中鼻甲前端附着点的平均距离为(1.91±0.32)mm。钩突的前上末端到中鼻甲前端附着点的平均距离为(0.31±0.10)mm,钩突的后下末端到下鼻甲附着点的垂直平均距离为(1. 16±0. 40) mm。筛泡中心到下鼻甲附着缘的最小平均距离为(11.96±2.31)mm。筛顶板长(27.22±6.91)mm,筛顶板前部宽(7.93±2.80)mm,中部宽(9.10±3.72)mm,后部宽(9.83±2.31)mm。中鼻甲前下缘距纸板最小平均距离为(9.68±2.12)mm,中鼻甲基板前内下缘处距纸板最小平均距离为(11.06±4.23)mm,中鼻甲前下缘距筛板的距离为(19.96±6.46)mm,中鼻甲基板处前内下缘距筛板的距离为(17.98±5.53)mm。④结论　筛窦解剖结构的径线,特别是与鼻腔鼻窦某些手术保留? Objective Based on the practical need in endoscopic surgery for the ethmoid, the safety landmarks of it were measured so as to provide reliable anatomical data for avoidance of severe complications during the procedure. Methods In this study, 30 adult cadaveric heads without any lesion were selected at random(male, 23; female 7) and split on the level between the line of superior superciliary arch above and 1.0 cm high of occipital bone with Model MJ 3435 band. The brain tissue was removed, and then split axially on midline, and the nasal septum was also removed. The skull bone between the infraorbital foramen and central section was split axially. According to the approach of endoscopic ethmoid sinus surgery, the meridian of the anatomical structures was measured and the variations of ethmoid observed. Results The distance from the central agger nasi to the attachment of anterior middle turbinate was (1.91±0.32) mm. The average distance from the anterior and superior uncinate process to the anterior attachment of middle turbinate was (0.31±0.10) mm,and the vertical average distance from the posterior and inferior uncinate process to the attachment of inferior turbinate was (1.16±0.40) mm. The minimum average distance from the center of ethmoid bulla to the attachment of inferior turbinate was (11.96±2.31) mm. The length of ethmoid roof was (27.22±6.91) mm ; the width of anterior ethmoid roof and intermedium ethmoid roof and posterior ethmoid roof were (7.93±2.80) mm, (9.10± 3.72) mm and (9.83±2.31) mm, respectively. The distance from the anterior and posterior middle turbinate to the lamina papyracea was (9.68±2.12) mm; and the distance from the anterior and inferior part of middle turbinate basal lamina to the lamina papyracea was (11.06±4.23) mm; the distance from the anterior and inferior part of middle turbinate to the cribriform plate was (19.96±6.46) mm; the distance from the anterior and inferior part of middle turbinate basal lamina to the cribriform plate was (17.98±5.53) mm. Conclusion
