摘　　要： 妇幼保健保偿制是中国卫生部门的一项创举。笔者及同事在云南贫穷农村地区进行了较大规模的保偿制现场调查研究,本文报告了部分调研结果:1.保偿制确实对基层妇幼保健人员起到了积极的作用。2.绝大部分妇女对保偿制所提供的服务表示满意或基本满意。3.许多育龄妇女尚未充分利用保偿制所提供的服务。4,第二孩母亲和家庭贫困的妇女更少地利用保偿制提供的服务。本项研究表明,在农村社区坚持不懈地促进社区成员自我保健意识的提高,增加所提供保健服务的程度,将是促进人口生育健康的一项带根本性的长期任务。 Maternal and Child Health (MCH) Prepay Scheme was created in the late 1980s as a strategy to maintain MCH services in rural China while the services are shifting from free - of -charge to fee -for service. This paper reports some surprising findings from a survey of a random sample of 1 616 reproductive aged women residing in villages of five townships in rural Yunnan on prepay scheme. Outcomes of focus group discussion and in -depth interview with key informants are also utilized. The findings show that a vast of local women tend to underuse the prepaid services. The main factors attributed to the underuse of prepaid services include: traditional culture beliefs; not being aware of prenatal and postnatal care; and not knowing prepay scheme at all. The authors conclude that given the vast variation in traditional culture and unbalancement in socioeconomic development in rural areas of China, a strong element of community participation and involvement should be incorporated into the scheme from its inception.
